Appointment Scheduling

All visits to our office should be scheduled, including same day and urgent care appointments. If you need to ask questions about your appointment or to change it, please call our office at 318-212-6270.

Required Patient Forms

New patients should arrive 30 minutes prior to appointment time so we can process the information, create the medical file and place the chart in line for the designated appointment time. You can complete your new patient paperwork online by clicking the links below. Bring all medications in their original bottles (not a pill box) to your appointment. Be sure to include any over-the-counter medications you currently take.

Our Location

We are located at 2551 Greenwood Rd, on the campus of Willis Knighton North.

General surgery is in the Medical Arts Building, suite 311.

Bariatric surgery is in the Medical Arts Building, suite 330.

You can access your information online using our Patient Portal.

Fees & Billing

All co-pays are due at the time of your visit.

We ask that you provide your current insurance information at every visit, to make sure we have your most up to date insurance information.

We accept cash, check, Mastercard, Visa, and Discover.

Pay Your Bill Online

You can pay your bill online using Online Bill Pay.

Insurance

WK Robotic & Laparoscopic Surgery accepts most insurance plans. Our staff will be happy to file the insurance for you. Your insurance coverage is an agreement between you and the insurance provider. However, the final responsibility for payment of the bill belongs to you.

Whenever you come to our offices, please remember to bring your insurance card and carrier information with you.

This will help us be more efficient as we provide the best care for you.
